OVERVIEW

Our Youth Bridge Outpatient Program is a beacon of hope for children and adolescents who are struggling with chronic or severe mental health issues. Our Family Based Wraparound treatment improves safety and stability at home, allowing youth to stay with their loved ones. Our program not only addresses the behavioral health issues that put them at risk for out-of-home placement, but also promotes positive development, resilience, and healthy family functioning.

Our therapists and care navigators work tirelessly to ensure that every participant receives the care they need. Through intensive wraparound services, including individual and family counseling, as well as targeted case management, we help our patients overcome their challenges and build a better future. We believe that every individual has the potential to lead a fulfilling life, and we are committed to empowering them to achieve their goals.

WHAT TO EXPECT

The majority of the services offered in the office setting and on an as-needed basis in the patients' homes. Our care navigators mostly provide personalized care and attention to patients in their own space, and we also offer the option for therapists to provide in-home services if they choose to.

Services are provided in the office setting, home and community for six months or longer depending on the severity of what is going on.

For the Bridge Outpatient Program, each youth participant must receive both weekly individual counseling and weekly family counseling. The program offers three different options for the number of hours per case per week: 60 individual / 60 family, 45 individual / 45 family, or 60 individual / 30 family.

The program works with the youth, family and other people who support the family to decrease symptoms and create a safe and stable home.

The youth and family are active partners in treatment. This is a key factor in maintaining success after leaving the program.

WHAT HAPPENS NEXT

The family will work with their clinician to schedule one to two sessions each week or more as needed. Hours for sessions are flexible. Session types, frequency and length are determined on a case-by-case basis to meet the needs of each youth and family.

The clinician and family will discuss safety planning across environments (home, school, community) to reduce risk factors and ensure a safe environment for each youth.

A psychiatrist referral can be made, if needed.

The care plan or next referral will be discussed with the family and other team members to assure sustainability of treatment successes after the program.
